Justice Department to enforce Trump’s ban on gender transition procedures for minors

Attorney General Pam Bondi has issued a directive instructing Department of Justice (DOJ) employees to enforce President Donald Trump's policies targeting gender-affirming medical treatments for minors.

The memo, issued on Tuesday, April 21, accuses the medical community of giving "misleading advice" to families seeking care for gender-dysphoric children and singles out pharmaceutical companies and hospitals for allegedly concealing harmful side effects.

In line with this, Bondi has directed the DOJ to draft legislation to restrict gender-affirming care for minors, form a state-federal coalition to oppose "child mutilation" and investigate medical providers and drug makers for purported misinformation about transition-related treatments.

"Pursuant to the President's directive, I am issuing the following guidance to all Department of Justice employees to enforce rigorous protections and hold accountable those who prey on vulnerable children and their parents," Bondi wrote in the memo.

Bondi has dismissed the stance of major U.S. medical organizations, including the American Medical Association (AMA) and the American Academy of Pediatrics (AAP), which endorse gender-affirming care as clinically appropriate and often life-saving for transgender youth. Guidelines from these groups emphasize counseling and, in rare cases, reversible puberty blockers for adolescents, while surgical interventions are nearly always restricted to adults. The AAP notes that nonjudgmental, evidence-based care reduces suicide risks among transgender youth.

Instead, Bondi wrote that the medical field risks have already become an "unconscionable ideology" rather than a safeguard for children.

"Protecting America's children must be a top priority, whether from drug cartels, terrorists or even our own medical community. Every day, we hear more harrowing stories about children who will suffer for the rest of their lives because of the unconscionable ideology behind 'gender-affirming care.' Under my leadership, the Department of Justice will bring these practices to an end," Bondi concluded.

Trump prohibits federal funding, promotion or support for gender transition procedures for minors

The directive follows Trump's January executive order, "Protecting Children from Chemical and Surgical Mutilation," which prohibits federal funding, promotion or support for gender transition procedures for minors. It also vows to investigate medical providers accused of obscuring the long-term risks of such treatments. In the executive order, Trump directed federal agencies to ensure that no taxpayer dollars support medical interventions, including puberty blockers, hormone therapy or surgery, for transgender individuals under 19 years old. The order also begins the process of stripping gender-affirming care coverage from federal healthcare programs, including Medicare, Medicaid and TRICARE.

Key provisions of the order include conditioning federal grants, contracts and reimbursements to medical institutions on their refusal to provide transition-related treatments for minors. Additionally, the Department of Health and Human Services (HHS) must reverse guidelines based on the World Professional Association for Transgender Health (WPATH) standards – standards relied upon by major medical organizations, including the American Academy of Pediatrics.

The order also calls for federal investigations into pharmaceutical companies and hospitals accused of concealing risks linked to gender-affirming care. It further urges Congress to pass legislation allowing lawsuits from individuals who later regret transitioning as minors, as well as their parents. To enforce these measures, the DOJ will collaborate with state attorneys general to crack down on what the administration labels "female genital mutilation" – a term controversially applied to gender-affirming surgeries for minors.

Lastly, by 2026, federal employee health plans will be barred from covering transition-related care for dependents under 19.
